Disclaimer:

• In submitting this form, I understand that the information will be used only in case of a medical emergency and to participate in Basketball Camp activities. I understand that Front Page Sports (FPS) does not carry any medical insurance for FPS Basketball Camp program participants. I recognize my responsibility, through appropriate insurance or otherwise, to cover all medically-related expenses if such circumstance should arise. In the event that my emergency contact cannot be reached at the number listed, I grant permission to FPS to arrange for transportation to a hospital and for medical services to be rendered. I understand and agree that not disclosing relevant information regarding any known behavior or health issues will be grounds for immediate dismissal from the program without a refund.

• I grant permission for FPS to photograph me during the FPS Camp and to use these photographs and video to promote FPS activities and programs.

 

• If I cancel my registration for any reason before the start of the camp I will receive a refund of my registration fee less a cancellation / service fee of $75.00 or FPS Basketball Camps will give me a credit towards a future camp. This credit will be valid for one year from the start date of the camp for which I canceled my registration. No refunds or credits will be given for any cancellations within one week of the camp. All cancellation requests must be emailed to frontpagesports.adm@gmail.com.
